PROBLEM
CATERPILLAR has recently investigated reports of cracks appearing in the metallic swivel access platform fabrication. This service letter reinforces all the details that were published in 2 TIB?s, TIBU4401 and TIBU4407. This issue affects both "A" and "B" SERIES Swivel Access Platforms. The part numbers affected are 212-1299, 234-9518, 213-6822, 213-6823, 213-6824 and 197-3944.AFFECTED PRODUCT

PARTS NEEDED
Order 1 SEHS7332 Tag per access platform.ACTION REQUIRED
All THP24S SWIVEL ACCESS PLATFORMS must be removed from service immediately. All swivel access platforms need to be tagged with a "DANGER DO NOT OPERATE" tag FORM NO. SEHS7332. The swivel access platform must be placed in a secure area away from the customer?s premises.Dealer to confirm through the service claim that the Swivel Access Platform has been removed from service and returned to a secured location controlled by the dealers.
This Safety PIP is the first step in a program to ensure safe working with Caterpillar Access Platforms. The program is aggressive and elements of the program will be implemented as soon as product is available.
Prior to making the service claim for each Access Platform all dealers must complete the "Customer Name and Address Details" into SIMSi.

Dear Caterpillar Product Owner:
The 212-1299, 234-9518, 213-6822, 213-6823, 213-6824 and 197-3944 Swivel Access Platforms need to be removed from service. The swivel access platform may crack and cause personal injury or death.
Contact your Caterpillar dealer to remove your Swivel Access Platform from Service.
Your Caterpillar Dealer will work with you on an interim solution for your access needs. You will be notified by Caterpillar when a final solution is available.
Please refer the dealer to their Service Letter dated February 1, 2005.
